What is Lipase?

Lipase is an enzyme that breaks down fats (lipids) into smaller molecules. In the gut, lipase enzymes are crucial for the digestion, transport, and use of dietary fats and oils [1].

Although there are various kinds of lipases, pancreatic lipase is the most important one.

It’s the main source of lipase in the gut, produced and released by the pancreas — the same gland that makes insulin. After a meal, your pancreas releases pancreatic lipase into your digestive tract, where it breaks down triglycerides from oils into smaller molecules that are easier to absorb (monoglycerides and fatty acids) [2].

Apart from the pancreas, a small number of lipases are also produced in the liver (hepatic lipase), blood vessel lining (endothelial lipase), and other tissues such as the muscles, heart, brain, and fat tissues (lipoprotein lipase) [3, 4, 5].

This article will focus on the main, pancreatic lipase: what it means when its levels are abnormal, how this affects weight loss and natural ways to decrease high levels of this enzyme.

Lipase Test

A lipase test measures the levels of lipase in your blood. When the pancreas is inflamed or injured, it releases more lipase than usual, which reaches the blood and builds up. Under normal conditions, not much of this enzyme makes it into the blood from the gut.

Doctors may order a lipase test if they suspect pancreas inflammation (pancreatitis). Symptoms of pancreas inflammation include the following [6, 7]:

  • Moderate to severe abdominal pain/back pain
  • Nausea
  • Vomiting
  • Loss of appetite
  • Oily/fatty stools

If you have these symptoms, your pancreas may be inflamed or injured and you should see a doctor.

You may be at risk for pancreatitis if you:

  • Have high triglyceride blood levels [8, 9, 10]
  • Drink too much alcohol [11]
  • Have been diagnosed with gallbladder stones (which may block the flow of pancreatic juice into the intestines) [12, 13, 14]
  • Have a family history of pancreatitis [15]
  • Have type 2 diabetes [16]
  • Are overweight/obese – this increases the severity of pancreas inflammation [17, 18, 19]

Normal Range

The normal range for a blood lipase test varies slightly between laboratories. Usually, it’s between 12 and 70 U/L (units per liter).

High Lipase Levels

Overall, lipase is most often elevated due to pancreas inflammation (pancreatitis). But it can also be increased due to other underlying conditions that inflame the pancreas (such as gallstones or gut issues), or even due to kidney damage; since lipase is metabolized by the kidneys, damaged kidneys cannot eliminate lipase and it builds up in the blood. Also, many problems with the digestive system, including sensitivity, inflammation, and damage can increase this enzyme [20, 20, 21].

That’s why the causes of high lipase levels can be so diverse. They include all the following:

  • Bowel obstruction, which happens when the small or large intestine is completely blocked [22, 21]
  • Gallstones and gallbladder inflammation (cholecystitis) [1, 21]
  • Stomach and small intestine inflammation (gastroenteritis) [1]
  • Abdominal lining inflammation (peritonitis) [23]
  • Stomach and duodenal ulcers [1, 21]
  • Crohn’s disease (IBS) [1]
  • Celiac disease [1]
  • Alcoholism [1, 21]
  • Liver disease or Hepatitis C infections [21, 1, 24]
  • Cystic fibrosis [25, 26]
  • Diabetes and diabetic ketoacidosis (breaking down fat too fast, which builds up acidic ketones in the blood) [1, 27, 21]
  • Kidney failure [28, 21]
  • Critical illnesses and organ damage/trauma [1, 29, 30]
  • Liver and gut cancers [1]

Lipase can also increase in some rare diseases, such as:

  • Sarcoidosis, an inflammatory condition that affects multiple organs in the body [1, 31]
  • Gullo’s syndrome, a rare hereditary condition with slightly high pancreatic enzymes and no underlying health issues [32, 33]
  • Macrolipasemia, a rare enzyme disorder in which lipase abnormally binds to antibodies in the blood (often IgG) [34, 35]

Some drugs can increase lipase blood levels, including:

  • Opioids such as codeine and morphine [36, 1]
  • Type 2 diabetes drugs sitagliptin (Januvia, Janumet) and saxagliptin (Onglyza, Kombiglyze) [1]
  • Neostigmine (Prostigmin), a drug used to treat myasthenia gravis, Ogilvie syndrome, and urinary retention [1]

High lipase levels are linked to many serious and life-threatening conditions. It is best to see a doctor as soon as possible. Your doctor will run follow-up tests to discover the underlying cause and prescribe appropriate treatment.

Low Lipase Levels

Low levels of lipase can mean that your pancreas is not functioning well and is struggling to produce digestive enzymes.

Some conditions that can increase lipase can sometimes also decrease this enzyme.

Low levels (close to 0) can be found in:

  • Diabetes, both type 1 and type 2 [37]
  • Cystic fibrosis [26]
  • Chronic pancreatitis [38]
  • Pancreatic cancer [39]
  • Cancer radiotherapy and/or chemotherapy [40]

If you suspect that you have low lipase or digestive enzymes, see your doctor.

Other people may have slightly reduced digestive enzymes in the gut that don’t affect lipase blood levels but can cause indigestion. Supplemental digestive enzymes can help when natural production is low.

Decreasing Lipase

To lower high lipase, you need to address the underlying issue triggering the lipase increase. This is most often pancreas inflammation. An inflamed pancreas is a serious condition that requires immediate medical attention — your doctor will diagnose the problem and prescribe appropriate treatment.

Other than that, when your lipase is high you should:

  • Avoid alcohol, it irritates the pancreas and damages it in large quantities [41, 42]
  • Keep hydrated (unless instructed otherwise), which helps with pancreas recovery [43, 44]
  • Make dietary changes or fast if necessary, which your doctor will outline [45, 46]

Blocking Pancreatic Lipase for Weight Loss


Pancreatic lipase breaks down fats in the gut, which helps to absorb and use them. Blocking the pancreatic lipase can reduce fat absorption, which aids weight loss [2].

Anti-obesity drugs that block pancreatic lipase are orlistat (Xenical, Alli) and cetilistat (in clinical trials). Orlistat reduces fat absorption by about 30% [47, 48, 49].

According to a meta-analysis of 33 studies (and almost 10k people), orlistat reduces body weight, blood cholesterol, and triglyceride levels [50].

Drugs that block the pancreatic lipase are relatively safe, but can still cause side effects.

Blocking the pancreatic lipase increases the absorption of oxalates, raising the risk of kidney stones. This is because more fats remain in the gut and bind to calcium, preventing calcium from binding to oxalates. As a result, oxalates build up in the urine and increases kidney stone risk [51].

Drugs that block lipase can also hinder the absorption of fat-soluble vitamins. Orlistat combined with a multivitamin reduced vitamin E absorption, and levels of vitamin D and vitamin K in a study of 17 people. Orlistat can also cause other side effects, such as oily stools, diarrhea, stomach pain, and liver damage in rare cases [52, 53].
